            The Court being advised that Senior Judge Michael Dennard, Reporter for the Standard Forms Committee, has requested the following adjustments to the Standard Forms Committee roster:  (1) Cheryl Meade, Terri Muse, and Jean Uranga be released from said committee; (2) Sara Staub, Michael Duggan and Scott Keim be appointed to said committee; (3) Judge David Day be released from the duties as Chair of the Standard Forms Committee; and (4) Judge Bradly Ford be appointed as the new Chair of said committee;

            NOW, THEREFORE, IT IS HEREBY ORDERED that Cheryl Meade, Terri Muse and Jean Uranga are released from the duties associated with the Standard Forms Committee; and Sara Staub, Michael Duggan and Scott Keim are appointed as members of said committee.

            IT IS FURTHER ORDERED that Judge David Day is released from the duties as Chair of the Standard Forms Committee; and Judge Bradly Ford is appointed as the new Chair of said committee.

            IT IS FURTHER ORDERED that after the above adjustments are made to the roster, the Standard Forms Committee shall be comprised of the following members:
